If you are handy, you can take the switch apart and fix it most of the time. Lots of little parts and springs. Good lighting, and a deep big box to work in. Only bend the tabs open enough to release the guts - they break off easily.
 
Russ, I just bought a 40 for parts. Coincidently I noticed it has a wiper intermittent switch wired in. I will post pics when time permits as an intermittent switch would be nice especially if its mass produced. Maybe its something that can contribute to your project.
 
Interesting, is it factory or aftermarket? The aftermarket stuff typically doesn't work on our wipers that switch on the ground side.

Here’s what I found on the donor truck. Seems like an in-line resistor. It was spliced in on the low speed side of the wiper.

I found this thread while troubleshooting my own identical gremlin and wanted to post for posterity.

From OP initial symptoms, mine are the same
- First click, nothing
- Second click, fast speed
- Did not park when turned back off

Solutions
Park: The wiper motor MUST be grounded to enable the park feature. My motor is good and this fixed parking for me.

No low speed: A MUST read for this, is @Coolerman Wiper Electrical Science article.

In here, he talks about a wire swap that Toyota did for the early (wipers above) wiper sub harness, LW/LB. The OP and myself have later (wipers below). This lead me to test the wire swap and sure enough, it fixed the slow speed. Further investigation showed that I had these incorrectly placed in the harness connector at the main harness.

Bottom line
If the OP motor was working, then he just needed to swap the LW/LB wires in that connector. I've confirmed this with 2 other late models and it fixed the missing slow speed issue. Here are pics of OP and mine.
