I have an HDJ81. A month or two ago my turbo, or I at least believe it is my turbo, started to make an odd clicking noise. Once I hit about 6 psi the clicking starts. I have checked the turbo to see if there is any axial or radial play and if the fan spins smoothly and all seems well- I did have the turbo rebuilt 3 years ago so I am hoping that there is nothing wrong with it. This noise doesn't stop once the truck is warm, hwy driving or anything, whenever the turbo is engaged above 6 psi it will make this noise to varrying degrees. I'd say exhaust leak at a manifold gasket somewhere as a first guess.
Boost leak as a second guess, but a boost leak will usually be a whistle, or sound like a cat's meow

If it's been doing it for a while, look for a sooty mark around the joints in the manifold as a tell rail for where the leak is. Maybe take heat shields off to get a better look
 
That sounds awful, and X3 on looking for exhaust leaks to start.
