I wouldn't expect the driveshaft bolts to be replaced - they typically don't "wear" and are safe to reuse indefinitely. It seems in this case they didn't torque them properly or didn't apply loctite. As mentioned, the bolts come from the factory with thread-locker pre-applied.
Man I tell ya one thing properly tourqed drive shaft bokts with red lock tight is one of the hardest damn things to loosen every curse word used heating does help . Blue lock tight is plenty.

Sounds like we've both been to this same place haha

Haven't had a chance to troubleshoot; I spent time inspecting everything and found the culprit.

Misdiagnosed; it turned out to be the rear driveshaft bolts. The plates were completely separated; all four bolts were loose and coming undone. I ordered myself new bolts and inspected all the other plates; they seem fine. No clue why the shop repurposed my old bolts instead of just charging me $60 for a new set. I'll do it.

The EXACT same thing happened to me approximately a year after a crate h55f installed on my truck, the work was done by a shop well-known on this web site. T case was rebuilt, drive shafts were removed, resized and balanced, all new u joints.
Began to experience horrible vibrations and rattling above 40 mph.
I nursed it home, crawled under the truck and found all 4 bolts on the rear drive shaft flange VERY loose, one nut was missing.
No evidence that loctite was used. Installed new hardware and applied blue loctite.
That was 4 years ago,no problems since.

this is just a case of telling the new guy to finish buttoning it up and he missed a bunch of shiit,

I went red for the drive shaft bolts, never gonna come off, ever, neverz..
I have my drive shafts off for one reason or another about every six months! 😂

Last time was when I got carried away and got the front axle airborne over a ledge on the Rainbow Trail (down closer to you). When I landed it twisted the front DS splines enough to lock up the slip joint. That was a rough ride home! If you ever need it, there's a great DS shop in Englewood.
