Hot exhaust is definitely the root cause of the issue. The inner nylon liner has a relatively low melting point, causing the deformation/delamination.
Unfortunately it looks like the original thread on this wasn't coming up high in the search hits for this as it sounds like several people went through the same issue I did after replacing the hose (having to replace it again).

When I replaced the hose for the 2nd time and fixed the exhaust leak with a new muffler, I also put some heat barrier wrap around the hose as a "just in case." It's been fine for over 4 years now.
