The brake pedal on my 96 lx450 is sinking to the ground and loses pressure. It does not happen all the time read a few threads but did not find an exact answer
 
Had same problem. Replaced the MC and it is all good. OEM Toyota part.
 
In the absence of any details to suggest otherwise, a bad master cylinder would seem the most likely explanation.
 
if you have a combination of sticky caliper pins on the rear calipers and loose rear wheel bearings you will have an occasional low pedal.

The loose bearings will allow the rotor to move in and out and the sticky pins will end up opening up the caliper. Then the first push goes into closing the caliper giving you a low pedal.

I had this problem and while offroading I did a rather aggressive hill climb and didn't make it. Coming off the gas and going onto the break had the pedal resting on the floor as the truck rolled backwards.

that left a mark!
