I'll say it, its gonna look janky but might work or it might not. I think a proper solution for the door or aftermarket bumper is the only way to go. Does your door have the encapsulated nuts you are just missing the bracket? You can sometimes find used parts for these on the JDM auto recycler part sites. Also, Isuzu Troopers from that era used a very similar door bracket for mounting the spare, even that could be a better option than a LR swingaway.

JDM Prado owners seem to like taking the spare wheel mount off the rear door. Aesthetics, probably.

If your truck has four captive nuts in the rear door, then it probably had a spare wheel mount. You can buy the mount and re-install it. This is the low-cost option, as it certainly costs less than that LR carrier.

Fabrication is involved with the kit you mention. It may require some work to function as intended.

If you want a swing-type tire carrier, get a Kaymar or something similar and do not look back. It does cost more but installation is easier.

I am also wondering this. I have heard that heavy wheels and tires can put a lot of strain on the door and mess up the hinges, but Toyota being Toyota it surprises me that they wouldn't have over-engineered it.

Something additional worth noting on mounting a spare on the door without a swing-out arm. The left door needs to open first, then the larger right door with the wheel on it can open. If the wheel diameter is large enough, it will extend over the left door and prevent it from opening - meaning you can't open either back door.
 
When I went to 33's, The tire was touching the bumper below. I figured that would help it stay on the truck if things began to sag.

As a general rule, if the spare tire carrier is in the frame, then the door is not reinforced.

Rear doors with spare tire carriers are reinforced.

How much fabrication is necessary for an internal tire carrier, I am not sure.

For 33+ tires, one option is to swing the tire carrier: this provides some clearance. Another option is using a wheel spacer.

If door hinge wear is a concern, then it is wise to use a bumper with a swing-type spare tire carrier.
