I have been struggling finding the name or part number for this rod end. The names with the relay rod are confusing. I need the rod end attached to the bell crank. Any help would be appreciated.
You can buy an aftermarket set of 5 (Brand 555, probably) from cruiseroutfitters. It's a little unclear, but I think you're asking about the LH Relay Rod End.
The number for the TRE appears to be 45045-69025. That number has possibly been updated. Toyota does some odd things and you might find it cheaper to buy the relay rod (is/was 45451-60040), with an included TRE, than buying the TRE alone. Perhaps @beno can look that up for you.
That would be the drivers side (DR) relay rod end at the center arm.

This might help on the nomenclature:
Tech Tip: While all of your steering link ends are often referred to as '"te rod ends", they actually have their own specific names. The rod that connects the passenger and drivers side knuckles/wheels is the tie-rod proper. The link connected to the steering box/gear is the drag-link and in the case of the 40 and 55 Series models, the link from the center-arm to the outer tie rod end is the relay-rod. 60 and 80 Series models don't feature a relay-rod rather a tie-rod and drag-link only.
I just ordered and received the parts, but have yet to install them, but I hope this helps. #57 that goes from the steering gearbox to the "center arm" (pictured) is the "drag link". #12 is the longest bar that runs parallel to your axle and connects the steering knuckles is the "tie rod connecting sleeve". #29 is referred to as the "center link" and is the shorter of the front rods and angles upwards to the "center arm". The "TRE connecting sleeve (#12)" has one long TRE with a hole to connect to the "center link" and one regular TRE. These are the thicker ones and are 21mm. The ends that go on the "drag link" and "center link" are all the same thread size and measure 17mm. One of them has a longer shaft where the castle nut screws on and I believe is referred to as "front, right" because it's the one that goes through the hole in the connecting rod. The other two should be the same and referred to as front, left. I ordered the TRE kit that had all 5. The two on the left are the thicker 21mm ends that go to the "connecting sleeve" and the three smaller ones are for the drag link end and the center link ends.
