The slave cylinder rod over-extending sounds very plausible to me. If you remember the root cause please share.

I want to say it was something weird like I hit the clutch pedal before installing everything, and not realizing the piston was almost out of the bore when I installed it, and compressing it again with a slightly crooked piston took a chunk off the piston. It blew out after only a few shifts.
