Front pipe sheared right off

I replaced my old front pipe with a new Walker front pipe in June. It sheared off about 6 inches below the manifold, just above the bend. this occurred about 150 ks from pavement, and got pretty loud through downtown Vancouver. I got a new exhaust manifold gasket from Toyota ( just in case), before I could see exactly what had happened. replaced front pipe with my old front pipe, which is still ok, but has beat up threads on the O2 sensor.

any advice as to why this happened, and for prevention. Brands or models for another one? Front pipe was clamped at the bell housing.

Yeah, I thought of that. I can't see a problem when I can look at it. I will get in there with a bar and try to move things. I did notice old pipe is a lot different design than the Walker. Was used hard off-road for the last 2 weekends.
 
Bought new motor mounts from Toyota. Replaced old ones in the pouring rain. Old ones didn't seem too bad, so I think the front pipe was just a bad design. At least I have new mounts, instead of 23 yr old ones.
 
I just threw the old y pipe back on. It still works, but the threads for the O2 sensor are a bit funky. Failed y pipe was a Walker. Much more flex in its design. Any different gearing for your 94 22re? What rear locker is it?
