Horrible way to go, and I feel bad for the family... but JFC that was a really f'n stupid way to rig.. and soooo avoidable with the least bit of common sense.

Pay for training folks! A $400 weekend course is nothing when compared to dying because you don't have a clue.

And for ****'s sake don't ever recover from a ball hitch, especially one on a 12" drop!! WTF...

A couple years ago I saw some video on YouTube from Ronnie Dahl in how that hitch ball comes flying like a projectile when you use it to yank someone.

He even demonstrated how it happens and with simulated straw men.

Anyways, never pull from a hitch ball
 
Not to be crass...but trucks that need a gigantic drop hitch (and is leaving it in while offroad) are not being surrounded by the sharpest knives in the shed.

If you have made it that far and need to pay $400 to understand basic 5th grade physics, then there have been many system failures.
