That will not fit with a Fairey overdrive on the back. The Fairey overdrive for the one piece transfer case does not interfere with the side mounted PTO. If you what for looking for you can see the Fairey PTO gear on one of the pictures.
I'm not disputing your knowledge that the pictures illustrate a Fairey overdrive.

Since power from the engine must transfer through the transmission in gear to get this device to turn. Transmission in neutral, no power going anywhere. I don't understand the OP's claim "The PTO has two gears but this added gear box allows both power-in and power-out without the transmission being in gear."

1. I didn't claim the rear-mounted PTO I posted is compatible with the Fairey overdrive. My point was, the OP pictures don't look anything like what the rear-mounted OEM PTO unit looks like.

2. The OP claims this device drives a winch. IF SO, how is it connected to the winch? Pictures, please.

The expeption to the transmission having to be in gear to drive the PTO is the FJ25 four speed. The PTO bolts to the side of the transmission. That one you should put the transmission in neutral when using the PTO. While I have never seen one the early four speed have an inspection cover that supposedly could power a PTO. Thinking something like a weather pump in a JDM fire engine.

Even the rear PTO still requires the engine to be running and transmission in gear. I always thought it was be cool if I had a rear mounted PTO to install a FJ40 rear axle in small military trailer. Then run a driveshaft out the center of the back slip mmjoint with long spline and U joint to the trailer driveshaft. By the looks of the rear PTO is does not change the gearing and rotates the same as the tailshaft. In high range would match. Low range would try to overdrivr the the vehicle. Something interesting to try when. Stuck in mud.

Some observations:
(1st part you already know) in 82 when Toyota went to the one way PTO gear box on the side of the transfer case they changed the PTO winch worm gear to wind the other way (I forget which way) so if you have a late model one way PTO gear box you must use the post 82 winch worm and wheel set so you get 5 wind in gears and one (slow in reverse) wind out gear (and not 5 slow out and one "fast" in as I did).
(2nd part) from the 82 change date the winch drive yoke from the shaft to the worm gear and the shear pin also changed, and at the same time the pin number went from 90255-04001 to 90255-04002, and it looks like the later pin is the same material but about 1/4 bigger in cross section, from about 4mm to 4.5mm. the Yoke change would be for the thicker pin hole. My guess is that Toyota re-did their sums and found that they could still be safe with a "more useable" stronger shear pin.
The parts I am working with are all old and very worn so if someone has good parts to measure I would like to know if my guess is correct, cheers from Australia

11/84 was the start of the 70 and the start of the one direction PTO. The few 40 series built after 10/84 still show having the two direction PTO. Not 100% sure if all 60 series changed on that date to many versions and markets to check them all. Never seen a listing for a 70 series with a two direction from the factory. Towards the bottom of the first page of this thread I posted a picture of three PTOs. The one with the shift lever on the front side was pulled from this 84 HJ47RV from Australia.

I have the PTO and driveshaft which is three shafts. The extra carrier bearing in the driveshaft started 10/82.
This was a treasure to find a short distance from my home. Got various parts besides the PTO parts. That includes the entire drivetrain minus the 2H.
