For whatever reason, when I pulled the ACC relay from the fuse box and re-seated it, apparently that fixed the problem. If it does this again, I'm replacing the relay.

Wanted to add that my 100 series had low voltage at each power outlet. Just removed and tested the ACC relay under the hood. When I installed the relay back, everything works fine again. Weird. Maybe a contact issue at the relay?

Same as you, if it happens again I'll be replacing that relay.
 
Same issue here, 01 LX470 - the cigar lighter in the middle of the center stack has power, but the other three don’t.
The fuse in the kick panel (fuse 46) was blown, so I tried to use the good fuse from the “cigar” (fuse 34) - no luck.
Relay under the hood is good.

Did you check the 50 amp fuse in engine bay?

Pulled up the owner's manual and do not see a 46 in the kick panel fuse box...Can you clarify which fuse blew? Also, what caused it to blow? Also any aftermarket wiring?

I was using this diagram:

Re: additional wiring, I have a Garmin dashcam running off the obd port, and some under seat LEDs running through the center console outlet. The LEDs work when plugged into the “main” outlet at the center of the center stack.

The 15a PWR OUTLET mini fuse was bad. Replacing it did nothing. I have yet to check any other fuses in the kick panel box, but all other systems are working afaik.
 
Check the one labeled Cigar shown on far left on the cover.
