Weird clutch failure

Got a call from my ex today, she was driving her truck ('94 V6 Xcab) up a hill, had to stop at a light. When the light changed, truck wouldn't move, nothing. Backed it down the hill, level ground, still wouldn't move at all. Odd semi-grinding sound when she let out the clutch. Hmmm...

Had it towed to my house, dropped the tranny this evening. This is what the clutch looked like:

The pile of scrapnel sitting on the towel is what is left of the disc that is supposed to be against the flywheel. It was shredded! The rear half was floating free from the disc center, which is the thing at top center in the photo. All the rivets were gone. Still plenty of material on the rear part.

That clutch is an OEM one, I installed it about 60K ago.

Any ideas why this happened? I've done lots of clutches, never seen an OEM one fail like this.

This is a person that has been driving manual trannies for longer than you young pups have been alive.

my only guess is that there was a defect in the way it was made. i dont think no matter how bad the driver is that any one person could cause that mutch damage
 
definatly a factory defect. take it back to toyota and show it to them if they are a standup operation at the dealership you got it at they will replace it for free.

looks to me like what happend was the rivets let loose on the back and basicly allowed 0 friction to happen on the back side of the clutch transferring a bunch of friction to the front sice of the clutch tearing it up.

just an idea who knows what really happend but that is a wild falure.
