I ended up using a hose clamp to connect to the clutch fork. Not pretty but it solved the problem. I took the truck to a muffler shop to see if they could weld a tab onto the clutch fork for me but they said it was cast iron and that it would end up flaking off.
I like the adjustability of the earlier slave too. If I had a later truck, I think I'd use the earlier fork and slave. That should work just fine. There is a little tiny metal bracket with the attachment for the spring-get that off a parts truck or order a new one from Toyota. You can visually adjust the slave in an older one-take off the metal clutch cover, and adjust untill there is 1mm of space between the fingers and the end of the throw out bearing. Then you know it's right.
