Were the weights taped on the inside of the rim or clipped to the outside? Having the weights outside would make it a quick assessment if it was the weights flying off. I know its stating the obvious but I would start there.

There are weights on the inside and outside so that makes it tough to determine. Also, I cant remember how many weights were on each tire since there were a lot.
 
If they are the stick on weights and whoever installed them wasn't careful and didn't tap them on with a hammer to ensure full adhesion and clean the rim with brakleen they can come off very easily. They could be hitting a caliper on rotation also if not fully adhered. Look for scratches on the weights or small particles of the lead on the caliper. With the heal lately I would highly doubt flat spots with BFG's on a stock 80 and even so they would go away as soon as the tire heated up.

You also mentioned that there are weights on the inside and outside. You should check to see if the weights on either side of the tire are dirty ie not new or that they are not obviously the same kind and age. Unless requested most places don't do a dual plane balance anymore which means there is the possibility that they left the old weights on (from Revo's) and just added to the back. Commonly done nowadays is to just add weights to the inside of the rim "close" to the center of the rim and call it good.

You should see what info you can gather from a thorough going over and take it back to the place you had mount and balance the tires. If they are at all reputable they should re-check the balance at no charge. It would cost them almost nothing to spin it on the machine again and throw on a couple bucks worth of lead.
