Does anyone know what the hose in the photo is called or better yet have a part number for it. It’s on my 2004 Land Cruiser V8 288,000 miles.

It should go to intake and definitely not be plugged. Allows clean air through valve covers to PCV on other side. It looks like it's in good shape.
On my 99 its called hose ventilation #2.
I would think you could use any inexpensive hose of same inside diameter.
according to my parts guide it's 12262-50080 Breather Hose.
