Hi:

KO3 275/65/18 .. can I use on my 2021 GX460 without any lift or spacers etc. Or should i goto 265/65/18 ? Kindly provide guidance.

The 275s are likely to rub somewhere. Above he hits the UCAs.
265s are also likely to rub too...probably fender liners. Easy fixes.

Every truck is different, so youll see a wide variety of "these tires fit" and "these tires rub" but your alignment can be a huge differentiator in where exactly the wheel is placed within the wheel opening.

If you are not really flexing it out fully offroad, then you can get more aggressive on tire fitment. If its taking you to the store or work only, then you might not need all it has to offer.
 
Yes not aggressive on dirt road or offroads, majority of the time will be driving on highways ( long distance ).

so in that case i can take care of fender lines ( like you said its easy to fix ). I just dont want rubbing on CUA which i read is only at full lock on the turns.

I often go full lock or close to it when parking. It took a couple rounds of heat molding the liners to get my 265/70R17s to not rub near full lock.
 
So the actual tire diameter will be ±32.1" O.D.
You should be fine with no other adjustments other than maybe at lock-to-lock.
Not knowing your budget, my suggestion would be to consider the following:
  1. If you go this route, I'd suggest at least a 1" suspension lift.
  2. 95% of the folks on here would whole-heartily agree that an after market suspension upgrade will absolutely improve ride quality, give a better look/stance and better performance on any, what so ever trails you ever go in with the added wheel travel and taller tire sidewall.
  3. Double check that this size will fit in the same space as your current spare tire.

Yeah, if its really only at full lock, a little rubbing might only just scrub the paint off the UCAs eventually. You could get aftermarket UCAs since they tend to have smaller ball joint areas depending on brand. If you are going to get aftermarket wheels something like a 0 or -6 offset might make for more UCA clearance.

In the end going with the 265 may be best. Id suggest getting a Standard Load or C-rated tire if they exist in the size you choose for a little less weight - will help not destroy your gas mileage further on those long distance trips.

Good point! The softer side wall will also give a better ride too!
 
I run 275/60/18s LTX M&S with no rubbing, but I installed a cornfed 1" lift in the front to level it a bit.. I care about the width more than the height, and when I installed 275/65/18 the proportions looked off.
