Can I assume that these block-off plates are necessary when installing an Exhaust Header?

No, not necessarily. If you have a cracked intake manifold you want it either way, with the stock exhaust or the header. If you don't have a cracked manifold, it certainly wouldn't hurt to have it there but your highly unlikely to crack an intake manifold with the header imo.

A few options. You can get away with running a single gasket (intake manifold - gasket - block off plate - exhaust manifold) and that will help with aligment (the same thickness of the gasket). However my preferred method is to use a die grinder to gentle massage those holes to 'center' them up. Some get far more mis-aligned than others. Hope that helps!
