HAM Radio Callsign - Personalized Plate

Does anyone know if getting your HAM radio callsign for your personalized plate is free or not? On the DMV site below it sounds like its free but on the reg17a form that you're supposed to fill out says its $21 first time fee and then no annual renewal fee. Which one is right?

I was always under the impression there was at least that one time fee.

Now which vehicle should I put my call sign plate on? Cruiser (I already have the call sign on my back window) or on the Miata.

BTW - good luck with all the BBW jokes that are sure to follow...again. We had fun when you first posted your call sign. Sure you don't want a vanity call sign from the FCC first?
 
A lot of HAMs are having an issue where the DMV puts a space in the middle of call sign plates and it looks weird, even lost plates are getting re-stamped with it.
 
I called the # that was on the link above and a manger said there is a one-time fee.

Yeah I'm sure about the vanity plate, it'll make my license plate interesting... :D
 
I was at the DMV this morning and I happened to remember to ask about the HAM operators plate. The nice, patient lady said that along with the one-time fee, there is a yearly fee for any plate except the standard issue plate. I didn't ask how much because she didn't look like she knew and would have to look it up and there was A MILLION PEOPLE behind me, I could feel them breathing and I wanted to leave that place!

Thanks! According to the form it's just a one-time fee. Sometimes I feel like the people at the DMV do not know what they're talking about... or at least some of them.
