Spray the old grommet with PB Blaster a few times over a few days; it should soften it up a bit, then get the longest pair of vice grips or pair of pliers you can find, then either use a hammer handle as mentioned above, or do what I did; use the engine lift hook for the fulcrum point and lift twist lift twist lift twist, etc until it comes out. The old grommet will crumble so have a pair of long needle nose pliers ready to pull the pieces out that fall onto the little shelf under the grommet inside the valve cover. Of course, don't do this until you have a new grommet in hand.

As noted, the key IMO is to use the engine lift hook as a fulcrum point. I used some huge pliers.
 
Looks like two years or less before the rubber grommet begins to turn to rock.
 
Old thread, but extremely helpful. I'm going to swap mine this weekend (88 FJ62) and plan to use all the tricks (big old pliers, bracing, PB, WD, warm engine, sacrificial offerings to the gods old and new.) Thanks for the help as always, mud!
 
A small pair of narrow but long needle nose pliers works well to pick out the pieces of the old grommet that fall into the oil baffle box. I now slather dielectric grease (which is a type of thickened silicone) over the rubber grommet, seems to help keep it pliable a bit longer. FWIW.
