I have the clunk. The driveshaft has been greased. A shop has replaced front outer hub flanges. This clunk is clearly the front axle rotating when I go from R to D. The clunk also happens while driving if I take my foot off accelerator then back on.

My question is from my (very limited) knowledge the front hub flanges should have addressed this no?

Video:

the splies that interface between the CV axle and hub flange are worn due to age and miles. You can replace the hub flanges, but ideally replace both CVs and flanges to make it back to no clunk at all

Yeah; both the CV axle splines and flanges wear. You need to replace both. Make sure to properly torque wheel bearings as well.

Is this the big clunk when shifting between D and R, or a soft one when releasing the brake pedal from a full stop?

In my search, after replacing anything that was worn, which for me was only diff bushings and sway bar bushings and links. it was rear brake pads and a proper brake bleeding that took away my random clunk after pressing on the brake stopping and then accelerating. The bleeding with new fluid did more than the pads in my case.
