By on top of the cat, do you mean between the cat and the engine, or the sensor is actually plugged into the top of the catalytic converter itself? I would think that the down stream sensor would be AFTER the cat, as it's measuring how effective the cat is at doing it's job...different
I meant to come back and update this thread. The spark plugs spacer did the trick for me and has worked consistently for over 2 years with no problems.

Passed but all 3 codes showed up after I drove it after 10 miles. Talked to a buddy of mine and he said replace your gas cap. $14 later and the codes all cleared as soon as I installed the new one.
The gas cap pressure test can mess up the OG gas caps...
