The hand throttle helps it running in the very first seconds of very cold idle. Otherwise it may stall because the engine is inhaling very cold air. Also, it make a hell of a difference if you leave it idling before taking off. The collant gets warm quickly and thus gives you cabin heat.
Ah, I see. Its the mix of very cold air and fuel that makes it difficult. There was a cold area pack for the 80 series for the northern countries which had among other things, an additional diesel heater which also helps with cold start. Mine happened to have it as a Denmark spec although I dont really need it.
 
I think that most people in a seriously cold climates are installing a circulating coolant heater, so the ACSD is going to be useless. While my HDJ81 (cold weather spec with heater on filter housing) did fire up in -20s F this winter, circulating heater is a game changer.

My personal opinion, I think the ACSD is ugly and I hate the extra set of coolant lines. I went so far as to rip out the coolant nipples and plug my block. Looks like it was never there...
 
Anyone has experience with very cold weather starting ability without the ACDS ?

My 1hfdt started last week at -29c (-20f) surprisingly well.
I'm worried that if someday the day comes where i may have to delete the acsd that my truck will no longuer start in very cold temp.
The oem 24V starter with intake heater and timing correction is simply amazing for cold starts !
No issues on three of mine. I add fuel by feathering the fuel pedal.
Note: Our HZJ75 and HZJ79 fleet never came with the ACSD, we glow and feather in cold weather.
