I am thinking the tps going into the intake and butterfly?
It does not look like it was tampered with. I did crawl under the vehicle and at some point someone had dented in the fuel tank considerably. Anyone think this could be related?

I'm not 100% on this, but if its not the AFM malfunctioning, and you have damage to your gas tank then maybe check fuel pump. Check the FSM if you have one or download it free from the sight. Then start testing the various connections with a volt meter. If you haven't already, test the resistance on the AFM plug and receptical. Good luck!
 
When you did the valve cover gasket, were the spark plug tube seals also done? Also the PCV valve grommet.
All these seem to shrink and cause huge air leaks to the system.

Worth a look. Check to see if the spark plugs are covered in oil (in the tube) If so, then the SPTS are bad.

Also can change the oil fill cap, or at the very least, replace the seal on it. They get hard and allow air to leak in as well.
 
Good information. I will double check the fuel pump. But it runs fine when under load and on the highway. Also spark plug tube seals were replaced as well as a Pcv valve and grommet. I’m just wondering if the tank was damaged and pinched a breather line if that could possibly affect it?

Is your Harness Wrapped Where it comes close to EGR? Known issue is cooked Wires.
Is your Harness Good Behind the GLove Box, another known wear point. Search is your Friend here.
Those are Gremlins that will have you pulling your hair out, but easy to rule out in 5 mins.
Another thing to try is Cleaning all the small ports in the Intake as they get gummed up.
Adding a Oil Catch Can^2 prevent buildup in future.
 
Are you getting any codes? I’ve recently had similar symptoms but I had a P171 lean. I removed the intake manifold and cleaned all clogged ports, replaced all hoses, cleaned my fuel injectors, new fuel pressure regulator and still had problem. Finally had someone put a gage on the fuel system and it was 25psi when it should have been 75 psi. Getting ready to slap a new fuel pump in and hopefully problem solved. Also check your main engine harness near the EGR valve. I found exposed wires but fortunately no insulation damage. Did a good re-wrap with high temp sleeve to protect it against future problems.
