Does it pop out of gear or just grind when going into 5th gear. If it pops out of gear or the gear shifter has a lot of movement in it, it is probably a shifter bushing. Otherwise what Kris said...
 
Does it have any troubles in other gears? Especially going from R to 1? That would indicate the clutch is failing.

5th gear is actually easy to get to in that tranny (assuming W56 series), you can actually do it with it still in the truck (but not as easily if you remove it). The overdrive gears & synchro is right in front of the transfer case.

Verify your clutch before digging into transmission internals.
 
yeah. sounds like clutch is not engaging completely. If clutch hydraulics are ok, may just require adjustment in pedal. I had to adjust mine a while back.
