I get hung up on the first page when the error message says "please use the provided phone format" but no info is provided and a couple of alternatives don't work. Help!

Cruise Moab 2009 will be held on
Wednesday, April 29 - Sunday, May 3, 2009.
I just put in for vacation time for this event, if approved, i'll be there.
