I would second the notion that one or more of the solenoids in the A/T is defective. I already experienced a similar situation wherein the A442F tranny shifts up only to 2nd gear and stays put no matter the speed of the truck.
What I did was to bring down the pan and check the electrical continuity of all solenoids. I found out that the 2nd solenoid (reckoned from the left side of the truck) was bad; it had no electrical continuity, i.e., its coil was open. The solution was simple: bought a new solenoid, P/N 85420-60050, and replaced the defective one.
Now the beast is back to its normal ways.
