What causes carbon build up???

low compression?
bad injectors?
valves need adjusting?

several things can add to it, oil getting into the chamber, poor fuel quality, incorrectly set up fuel system (where all the fuel is not getting burned up) and bad injectors are the common ones. Usually it's visible by a lack of performance. So all of your points can add to it.
 
my engine has never ran or started the same after i ran it out off fuel.

plugged injectors im thinking???
 
change the fuel filter, might want to add some cleaning agent to the fuel (like seafoam or others). Bleed the system well and try it out again. If it's the same you might have to have a look at getting the injectors rebuilt. You could pull a couple out and test them. The Factory Service Manual does have a test procedure for most injectors, one that doesn't need a large and fancy machine.
