I am torquing down the hub nuts and have one extension/adapter but to torque down the 48 inch pounds i need to use another adapter. Is there a torque difference? The guy at sears said about a 5 inch pound difference.

Some of the torque will be absorbed by the extention, the longer the extention the more torque will have to be applied to reach your 48 inch pounds.

Try not using an extention on a similar size bolt or nut and torque it to 48 in/lbs, then use the extention that you will be using and see how much more force you will have to apply. This will "calabrate" the torque wrench for 48 in/lbs.

Just my 2 cents

This sounds like a sound idea but unfortunately I don't have anything similar to the 54mm nut. does it have to be the same size of nut/socket?

It does not have to be the same size socket/nut/bolt its all based on the wrench.
