I decided to take my factory roof rack off and under one of the pillers there was rust. Do I need to just sand it down a little and re paint

You need to get to fresh metal no rust remaining. Then seal and paint. Are you putting the rack back on? Or deleting it?

Than you need to either weld The holes shut or plug them with rubber washers and bolts.
Yes, sand down to bare metal. Primer and paint it well. I used stainless steel bolts and some product over the top of the bolts to permanently seal everything.
My method held up great for 6 years before I totalled the rig.
