I'm working on a 1994 T100 3.0 with manual.
It needs a new radiator. PartsGeek has a Denso but it says it's for a automatic. Is the only difference the ports for the cooler lines? Or do I need to get one made for a manual?

Just pulled and tore down the 3.0 for a complete rebuild
 
I don't think there is a manual specific radiator available anymore. If you look on RockAuto's web site, a couple of the options show a diagram with a few measurements. You might want to compare them to what your old one looks like. I guessing they will fit, you just have to be sure to cap the transmission line fittings. Considering the age and scarcity of the T100, I imagine the radiator manufacturers have gone to the one-size-fits-all route.

I'd think as long as the inlet/outlet are the same side, it will work. And I know Toyota didn't change that.
The unused transmission cooler ports don't really need to be capped other than to keep mud or trash out of the cavity. It's seperate from the radiator itself.
