Good progress, the throttle body gasket is a non-reuseable part, part # 2227150050 - inexpensive and will help you get closer to solving your problem.
 
*Update*
After the above throttle body was cleaned, it seems it runs better; however, I am still getting the misfires on startup (fuel trims look better). Once the vehicle warms up and driven, I have encountered no issues.

Based on what @OEMGUY0720 has suggested, I could have a cat problem (I am pretty sure these were replaced some time ago before my ownership, I need to investigate further) or a vacuum problem still. I have looked at all these lines and I am not seeing any issues there.

My thoughts...
Could this be the secondary air-injection system causing problems on startup? It seems this is finicky problem on the VVTI. Getting no codes for this but that doesn't necessarily mean there couldn't be a problem.

Fuel injectors? This is more where I am leaning. Reason being, I had the hood up and was listening to the engine on startup, and the normal clicking you would hear from injectors randomly stopped and started. Could it be a clogged filter as well? I dumped some Techron fuel injector cleaner the last time I filled up. I plan to inspect further and ohm out the injectors, so more to come here.
