LC 200, Engine Bore worn out

Hi everyone. Need some expert advice, as in just 35,000 kms i developed a knocking sound from the engine. Initially thought that it was the injectors but once i opened up the engine the bore was scared and pistons had wear signs on it. Out of 8 x bores, only 3 x bores are fine. Need some guidance as to what could have gone wrong in taking care of the vehicle. It is 1VD-FTV (diesel) Twin Turbo (Armored) and 2016 model.
Would be anxiously waiting for some technical advice.

Were oil levels checked frequently enough to know it wasn't low oil level (after a change perhaps, filter leaking, etc.)? How about possibility of anti-freeze getting in the oil? I have read that can do big damage, but no personal experience to verify. The Australian websites frequently comment on the amount of dust the air filter system lets go through without some extra effort on an owner's part. Was the vehicle driven where a lot of dust may have been ingested?

Is there anything else you can tell us about the use, maintenance, and behavior leading up to the knocking?
 
It usually ply only on carpeted road, never gone x-country. All the maintenance was done as per the Toyota instructions. We have generally hot and humid climate therefore take extra care about the preventive maintenance. Dusting issue might be there but very unlikely, we have EURO 2 fuel in our country, hope that has nothing to do with the issue.
 
Most folks here on ih8mud are from the US, where we get the 5.7l petrol engine. So you might not get a lot of information here that is relevant to your engine.
