Is this another Radiator issue ?

Update:
I Ended up changing the top
Plastic part with an aftermarket one. The radiator shop mentioned that the radiator is still in good shape.
At what temperature does the thermostat usually kicks in ?

Stick it in a pot of water, bring it to a boil, and watch it. Should kick in around 180F. If I monitor my temp via the OBD2 port the truck runs at 195-196F all day long.
