A hose blowing off twice isn’t a clamping problem — it’s a pressure or a fitment problem. And the fact that it’s the lower hose matters: that’s the suction side feeding the water pump, normally the lowest-pressure point in the system. For it to let go there, something is way out of spec.
Two prime suspects, in order:
1. The new radiator’s neck. The timing points straight at it. Cheap aftermarket radiators often have hose necks that are slightly undersized, shorter than OEM, or missing the raised retention bead at the end that the hose actually grips behind. Clamp pressure alone won’t hold a hose on a smooth plastic neck. Have him compare the new neck to the old radiator side by side — OD, length, and whether there’s a bead. If the hose slid on easily, that’s the answer.
2. Combustion gases in the cooling system. A head gasket leak spikes pressure far past the 13–16 psi the cap is rated for, and it does it in pulses the clamp can’t resist. A block tester (the blue-fluid chemical kit) at the radiator filler takes ten minutes and rules this in or out.
Also worth checking:
• The radiator cap. If he reused an old one or the new radiator came with a no-name cap that isn’t venting to the overflow, pressure has nowhere to go.
• Trapped air. The 1UR-FE needs a real bleed procedure. Air pockets cause localized boiling and pressure surges.

OEM radiator takes fitment off the table — which pushes this hard toward an over-pressure problem. Something is putting far more than 13–16 psi into that system, and the lower hose is just the weakest joint.
Top candidate now, and it fits the story unusually well: the internal transmission cooler in the radiator. The GX460’s radiator has the ATF cooler built into the bottom tank. Transmission line pressure runs many times higher than cooling system pressure, so if that internal barrier is breached, ATF gets pushed into the coolant and pressurizes the whole system every time the truck is driven. That would also explain why the original radiator was leaking in the first place — and if the old one failed that way, the trans cooler lines may have already been contaminated, or the same underlying issue is still present.
Easy check: pull the radiator cap when cold and look for pink/brown froth, a milkshake film on the underside of the cap, or an oily sheen on the coolant surface. Then check the ATF for coolant contamination.
The other two worth ruling out:
• Head gasket / combustion gas intrusion. Block tester, ten minutes, cheap. Do this one first since it’s the expensive answer.
• Radiator cap. If he reused the old cap or the new one isn’t venting to the overflow properly, pressure has nowhere to go. Cheap part, easy swap.
One more thing to have him verify: that the overflow hose from the filler neck to the reservoir isn’t kinked, cracked, or blocked. A blocked vent path turns a healthy system into a bomb, and it’s the kind of thing that gets disturbed during a radiator swap.
If the coolant looks clean and the block test is negative, I’d suspect the cap or the vent path before I’d suspect his clamping again.
 
Appreciate the thorough response.

Nothing looked oily in the coolant when I had the system bled. When it pulled the cap this morning.

Overflow hose is routed in the same clips on the radiator as previous and is in good shape, not kinked.

I could see how a blown trans cooler in the old radiator could have caused it to fail. I find it difficult to believe the new radiator would also have a bad internal cooler. The coolant looked good when it drained but I can check the Trans fluid.

I think my next step is going to be to fill with distilled water, pressure test the cooling system and pick up a block test kit.
 
Maybe I missed this - but did you replace the radiator cap? If not I'd replace it ASAP. It should vent of excess pressure long before a hose blows off. If you did not replace the clamp on the radiator hose, I'd replace it too (FWIW I always replace the spring clamps with worm clamps - although many purists don't). Those spring clamps eventually weaken/loose tension after going through thousands of heat cycles.

While a blown head gasket shouldn't cause a radiator hose to pop off by itself, it would vent combustion gases into the system that could blow a hose off if 1) the radiator cap isn't working properly or 2) the hose isn't properly secured.

Also I'd suggest a vacuum fill/leak detection kit for the radiator. It will tell you right away if you have any leaks and will fill the system without air bubbles.

Lastly, a combustion test kit won't work unless you have some miles on the coolant (thereby allowing combustion gas residue to accumulate in the coolant). So it's possible you'll get a false negative from that test.
