I have a '91 with a 4.0, 187K miles. I was told that it is normal for my engine to sound like a large sewing machine. Is this true? if not what should i check?

When you say "sounds like a sewing machine" it absolutely reminds me of my 92. I want to say a valve adjustment might be in order, but I don't have any experience with that, so I can't give you a definitive answer.
 
pushrods, lots of crusty vacuum hoses and belts, and an exhaust gas recirculation system will do that.

a tune up consisting of new sparkplugs, plug wires, distributor cap and rotor, pcv valve (and maybe all new belts) and a valve adjustment will quite things down nicely
