I have a 2005 4 Runner and just noticed the other day this rust underneath my truck... Thoughts is it anything I should be concerned about? So I used some special paint on it? Thanks!

Looks like its mostly surface rust on the axle. Id was any lose dirt off, let it dry and hit with a wire brush, rust neutralizer and some good spray paint.
