A pre-amp boosts the incoming signal from the antenna into the radio... for pulling in.... weak signals. 
Bit it also boost back ground noise and any interference from the vehicle ignition. So... It is helpful, but not a real big deal except in low static situations... if your rig is off and you are sitting in the hills out of town at night trying to hear someone who is way out of normal range... then the pre-amp is the ticket.
Driving down the trail during the day.... not a lot of benefit usually.
In my experience anyway.

THanks mark. I was surfing CB info and couldnt figure out what the purpose was because as you stated, I thought it would pick up on the background noise while driving.
