To the 200 braintrust what is the biggest 18"x tire size that fits in the spare well? I recall seeing the debate on another thread, but my bad and could not find it.

Nitto Ridge Grappler 285/75R17 (33.8") fits but there's not much room left. Anything above 34" is probably right out.
Did you have to modify anything?
Nope. There's maybe 1/2" of clearance between the tire and the receiver hitch, so not a lot of free space. I still have clearance to the axle, but I don't think you could fit a tire approaching 35" under there.
FWIW the dealer swapped the tires for me and they left the little bracket which it there to keep the OEM spare from moving around. So they literally didn't modify anything. Whenever I drop the spare myself I will remove that bracket - it's just 2 bolts and it's totally unnecessary and ends up making the tire hang down an extra 1" or so near the diff. You can leave it or safely remove it... up to you but personally I'd remove it.
295/70r18 will fit if you remove the curved metal bracket on the front crossmember.
Any taller and you will risk rubbing on the track bar.
