Reporting back month later:

Still running very good! Idle is still high. It has gone down to normal a couple of times. Gas mileage is bad at 9 or 10 due to high idle. When I say high I mean 1000 to 1200 after warmed up in idle, so NOT crazy high like 1600 plus.

In the spring I'll check the temp sensor and throttle sensor. Some sensor is giving bad info to the ECU because the idle will start to come down then will kick back up.
 
I always seem to come back to the philosophy of "don't fix whats not broken".

Not fixed and too darn cold to check what's making the high idle. I'm pretty confident in the spring I'll do some electric meter tests and figure out what's not within spec and replace it.

"don't fix whats not broken" what? That's no fun!
 
Progress report 1/26/14

Not resolved at all and I think it's getting worse. Warm weekend, so tinkered a bit.

HI IDLE:
Last fill-up got 7.5 MPG. Yesterday I put the other (standard) AFM back in (EFI fuse out while I exchanged them), warmed it up, still 1200rpm high idle. So I tapped on the throttle body and idle went down to 800rpm.

Throttle body gasket: Toyota doesn't make them so I had made one and put it in. I figured I'd left too much material so today I replaced the homemade gasket (left) with an aftermarket (right).

When I did this, I was amazed at how dirty the throttle body was! Seems like oil is getting into my intake badly. I just installed a new PCV valve.

Cleaned it up with new gasket. Put it together, changed the oil (ready to run).

Again, it's running good except the high idle and black stuff coming out my tail pipe. I had to of screwed something up when I did the engine work in summer of 2011. Here's a pic of the ground next to tail pipe.
