Just noticed that my second row window (back seat) on the driver's side isn't working - checked both buttons (driver's and back seat) and made sure window lock was off. What might be the cause? Fuse? Thanks for any help!

I will rule out fuse. If the fuse is out, all your window will not work. You might want to check your master switch or the single window switch and see if the contact is bad or burnt. If not check your window motor or anything lose or stuck inside the door preventing the window from sliding. good luck :)
 
Try this for a quick and easy fix, open the door and whilst holding the window down button slam the door (really hard), you'll find with power windows that if they aren't used often the window sticks to the seals and the motors can't overcome the static friction. Good luck, hope this is of some help.
 
If slamming it doesn't fix it, you'll need a voltmeter and access to wires for both switches. It could be either switch and it usually is burnt or dirty contacts in one of the switches. the switch that isn't being used still has to pass the source voltage with it's NC (normally closed) contacts, so it prolly best to clean all contacts in both switches.
