IMO, the roughness/noise comes from a multitude of things. The 3UR has just one piston size... Toyota claims the honing process is now accurate enough for this. All UZ engines had three or four sizes that could be selected to fit the bore. Since all UR engines have gone to a one size piston process, and many are quiet, there is some doubt as to whether or not this contributes. With that said, it should be noteed that all other UR engine have a much shorter stroke than our 3UR. Another biggy, and my new favorite, is that the 3UR doesn't utilize right and left pistons. All othe UR and UZ V8 engines have right and left pistons. With R & L Pistons, the pin is usually offset to reduce thrust loads and piston slap. Skirt design may also be different. Add chains, 32 cam followers, and an aluminum block... We get more noise.
I can't say that I notice it. I suppose that is because I typically drive with the windows closed. I will say that the interior is far quieter than my old '03 V8 4Runner, which had the same 4.7l V8 as the 100 Series.

Some great replies here - thanks! A 2016 just popped up locally as a trade in with 4k miles on it, so I went to look at it today. I have to say this one was MUCH quieter and smoother running than the other 200s I've driven, so it seems there is a range of what kinds of noise levels these things emit. I was impressed by this one, but it didn't feel as quick as the 2008 I drove about a year ago which could be the additional weight from added options and perhaps even the 8 speed transmission?
A few other things bug me at this price level of over $70k - no passenger lumbar support, no power tailgate, and the most boring key/remote I've ever seen for a rig costing over $30k. The Nav/info screen is far improved over the 2015 and older models, and the materials and button layout was better too. Also, the view of the new hood from the driver's seat is great - the deep center scoop is pretty awesome and looks very cool. I'm not sure it's worth trading in my paid off Macan S and paid off Yukon Denali for it, but it's tempting. I'll think about it over the weekend and maybe take the wife to see it Monday. She has told me many times she misses the 100 series, but she also loves to take the Macan to yoga and girl's nights!

get rid of one (the yukon), keep the other (macan)... not even the sturdiness/reliability of a land cruiser can compete with two cars simultaneously! to be fair, i think it could take on either one by itself and win in a steel cage match - unless you're bringing the keyless entry into the mix. it's like kryptonite... the fob is a POS.

I wanted to update this thread in case it comes up via search later. I drove a couple more 200s: an 09 with 130k, a 14 with 45k, and the one I bought which is a 14 with 32k. I have to say that all of these drove smoother and sounded quieter than a few of the rougher ones I drove previously. The 14 I ended up with is incredibly smooth and even quiet on startup that honestly feels/sounds better than the 16 demo I drove. It seems there is a bit of variability in terms of noise and smoothness in these motors from truck to truck, but it seems I just found a couple not so great examples early on. Either that or I'm just getting used to it
