1HDt Turbo oil connections

Does anyone out there have, or have any idea where I can get a replacement part for the oil inlet/outlet gasket and assembly that connects the stock turbo to the engine. I don't have the part #, basically small metal tube from the block into the turbo, and then a metal return line that connects to a rubber tube.

Mine failed a month or so ago, 8 liters of oil on the coquihalla and a near end of engine experience. Had the peice repaired and it is failing again.. Any chance you can help me out, I am really desparate!

15407-17010 is the main number, it subs to 15407-17011
Pipe Sub Assy, Turbo oil inlet 9001-9501 1HD-T
gasket is 15471-58010 should be available from your Toyota dealer.
