rpm noise

2004, 370k miles. I just purchased it this Spring and have caught it up on all maintenance and repairs. Truck seemed well taken care of, no leaks, no smoke on start up, does not use oil, etc…. A little background, I have done all fluids, filters, Denso plugs and all 8 coils, new O2 upstream...
