Today putting in new plus noticed throttle butterfly dirty at edges. See picture.
Is tis normal (200K miles and ever cleaned) or does it indicate I need a full throttle body clean?

Mine looked like that during baselining at 150k. Sprayed some CRC throttle body cleaner on it and wiped gently with a lint free cloth. Clean as a whistle now. That looks pretty good for 200k IMHO if it's never been cleaned previously.
 
Defiantly clean that. Without warning you truck won't idle because of the dirt not letting it move as intended. While there do the MAF sensor. Easiest job you can do.
