Ive always felt measuring off the hub center to be slightly more vague. The ground is the ground.

Tire size and pressures are different across the board. The hub eliminates all the variables so its a good standard.
 
But the ground isn't really the ground, given the variables I mentioned. For your own purposes, you'd be right in that it's easier to measure ground to fender. For the sake of sharing here with questions about suspension set ups, adjustments, etc, the standard is hub center to fender so it translates the same to all trucks regardless of tire size. It's apples to apples when referring to suspension. Different strokes.
 
FWIW my wife has a brand new (0 miles & normal psi) 285/65r20 ko2 mounted in the spare tire location with zero issues. BFG specs it at 34.5” but BFGs do run small.

@sonk76 convinced me it would fit after he ran his previous 295/70r18 in the spare location

Three factors have me hesitant moving from 275/70/18s to the 305/65/18s... still on the fence:
1. Weight: My research (5 min. googling) shows an additional 5 pounds per tires - 57.7 to 62.8; not where you want weight, but likely not a noticeable change
2. Cost: $250 vs $317 per or $1,000 vs. $1,268 for four
3. Cost - and would appreciate some input here: If I stick with the current size, I can use the best leftover tire as the spare and only buy four tires. If I go to 305s, I'm thinking I'd need to buy a 5th spare - to keep from blowing up the dif' because of that roll-out difference. That's a valid assumption... right? That makes the cost difference $1,000 vs. $1,585

Stick with 275/70/18. Saves money and weight. I've been on 275/70/18 KO2 for 2 years and gone over and through everything I've wanted to without a single issue. Can run 18psi for off-road. On-road, the slightly narrower profile makes steering easier. Vastly more options available as well as 275/70/18 is a fairly common size. The 305/65/18 doesn't gain any appreciable advantage over the 275/70/18 IMO unless lightening your wallet is a goal.

The reason for going 305 was purely aesthetics for me, truck will more than capable with 275s, unless you're looking to beef up your stance a little, stick with 275 to make it easier for you. I love how mine looks though, that poke.

Needs new tires soon. I have tundra wheels and I’m contemplating 275/70r18 with a spacer (allows me to use oem caps which I have off currently…looks kinda s***ty) . Every tire out
There comes in this size. Alternatively, was thinking 305/65/18. Available in ko2 which is what I have and like.

Aesthetically, would these setups look the same on my truck

No, one is a 10.5-11" wide tire and one is 12". I ran 275 for a while and it performed fine but definitely doesn't compare to a full 12-12.5" wide tire.

Would a set of spacers and sticking with your current 275 size give you the look you want? Asking for a friend :)
