Steering box play adjustment coming loose (1 Viewer)

So, my steering box play adjustment lock nut has started coming loose. I rebuilt the box about a year and a half ago. All went well and I haven't had any issues with the adjustment lock nut coming loose until recently when it started dripping.

I reset and tightened it several times, but it kept coming loose after a few drives, so I removed the nut, cleaned it all really well, and replaced the crush washer. I also put some blue loctite on the nut, but I didn't think it would help much because I believe the adjustment screw is turning with the nut. It stayed tight for a few days until half way through a trail, I found myself snugging it again. I am tightening it pretty tight - as tight as I can without doing something stupid.

Anyone else have this issue and solve it?

Possible that the sector shaft bearings may have slop, allowing the shaft to move in and out and knock the screw and nut loose?
