I know there is surface rust and rot, and I know you can get rid of surface rust pretty easily. I am wondering if it is possible to get rid of rust that has pitted the metal, or if it's too late at that point. If it hasn't gone all the way through, does it still need to be cut out and replaced, or can you just wire wheel it and get the rust 'out'?

I'm currently going through the same process. I decided early on, "When in doubt...Cut it out".

Where are you in Louisiana? I'm currently in Lafayette, wishing I was back in Swan Lake, Montana...
 
Unless you are an accomplished welder, cutting and welding a truck frame can make the situation much worse. The pitting in your photo doesn't look all that bad to me. I would treat the area with phosphoric acid, let it wet-soak for a while, then wash off the sticky acid residue. Wipe with lacquer thinner and shoot some good catalyzed epoxy on the area. Let someone 40 years from now worry about it after that.
