Power delivery problem? Hum, hum, hum, hum, hum power, 3L (1 Viewer)

Hi, vehicle as per username, a 92 Landcruiser (Prado?) LJ79 with a 3L (2800cc Diesel) engine. Around 1300rpm, on medium load in a high gear (generally most noticeable on 4th gear going uphill) the power delivery is jerky, what could be best described as hum, hum, hum, hum, like I was playing around with the throttle on and off. NOT the knocking of too high a gear I may add... it's smooth enough, but just like I was putting my foot down and taking it off rapidly...

It's not THAT bad, but noticeable and annoying. I can drive through this, by 1,600 it's gone, or drop a gear and its over very quickly/unnoticeable.

Any ideas? I got asked about the fuel filter, well its just been replaced so that is out, I guess its a bit more than that.
