Also, either you got some sort of thread locker or junk on your wheel stud or it’s stripped.
 
I did. I picked up some from Toyota this morning. Everything is back together except for the retaining pin

It’s thread locker. Some kind of red loctite

Threads look like they are in poor shape (regardless of thread lock).

I'd give serious consideration to putting in a full set of fresh wheel studs while you have the hubs off the truck.
Agreed. That wheel stud looks like someone treated it like a torque-to-yield-and-keep-torquing
 
Wheel studs do look stretched!
 
your also going to want some new wheel studs and lug nuts

As long as the cone washers are fully seated after torqueing the nut. By the looks of it those threads do not appear to extend into the hub deep enough for the cone washer to seat. Also the studs that I recently bought from Toyota do not look anything the new one you show (maybe I got old stock?). They look exactly like the studs from the factory. Below is what I purchased for my 91.
42323-60010 Washer, Cone (For Rear Axle Shaft)
90116-08325 Bolt, Stud (For Rear Axle Hub Rh)
90201-08042 WASHER
90170-08204 Nut (For Propeller Shaft)
